Application requirements for:

Pre-Kindergarten through Fourth Grades

  • Application with $75 fee
  • Release Form (if applicable)
  • Parent Confidential Statement
  • Assessment - Once an application is on file, we will contact you to schedule a January assessment date.

Fifth through Twelfth Grades

  • Application with $75 fee
  • Release Form
  • Parent Confidential Statement
  • Student Questionnaire
  • Interview with Division Head - Once an application is on file, we will contact you to schedule a January interview appointment.
  • Independent School Entrance Exam (ISEE) - The St. Mary's school code is 431462. We recommend taking the ISEE in December or early January so we can receive the scores as close to our application deadline as possible.

Application Process

Applications for all school years are welcome at anytime, but preferably no later than January 15 of the year for which are applying.

Apply online - Mastercard and Visa are accepted for the online, non-refundable application fee of $75.

By Mail - Download the application plus the appropriate forms below. Complete and mail them with your check or money order of $75, payable to:
